Nurse Case Management Eldercare Services

Case Management

Geriatric case management, also known as care management, is a collaborative approach to assessing, providing, coordinating and monitoring healthcare services. When you come to OU Case Management, a care manager will work with you, your family, physicians and other health care providers to ensure that you receive all the necessary services to promote health and maximize your well-being while helping you to remain independent. The care manager serves as an advocate and care coordinator for all your healthcare needs, helping you and your family navigate the complicated health-care environment, decipher the often-complex medical language and understand all of the facets of your illness.  This includes aid in understanding and coping with medical insurance and emotional issues. A care manager can remove the burden of oversight of sick and/or aging parents or loved ones.

A clear, individualized plan for coordinating your health-care will be drafted based upon your goals, needs and desires.  Case Management oftentimes includes:

  • Preventing unnecessary or repetitive medical care (Disease Management)
  • Evaluating your ability to remain safe and independent in the residence
  • Obtaining proper medical care and securing necessary medical supplies and equipment
  • Obtaining home care nursing services
  • Obtaining assistance with homemaking and personal care needs
  • Coordinating service delivery and necessary medical follow-up
  • Providing information and education about community resources and residential options
