An enrollment form must be completed and submitted before you can be enrolled. All enrollment forms need to be uploaded into your Student Self-Service. There will be an "Enrollment Form" option on your To-Do list. Please do not email enrollment forms. Any forms received via email will be asked to be uploaded into your Student Self-Service. To expedite the enrollment process, ALL BSN STUDENTS, will complete a singular enrollment form at the time of admission/Orientation. This singular enrollment form will allow College of Nursing staff to enroll students each semester through the duration of the program. If a BSN student takes a leave of absence or is placed on an alternate completion plan, the student must complete and upload a blank alternate plan enrollment form each semester.
To ensure proper form filing, please refer to the How to Complete the Enrollment Form instructions. Any form not properly completed will not be accepted. All students must submit an enrollment form regardless of their program.
How to Upload the Enrollment Form instructions
To upload your enrollment form off campus students must register the DUO security authentication first: https://apps.ouhsc.edu/duo/. From there you can go to your Student Self-Service.
