Celebrating 20 Years of Service to Oklahoma Communities
The University of Oklahoma Health Sciences Center Fran and Earl Ziegler College of Nursing Case Management program began in 1995 with a small group of faculty in Oklahoma City. The OU Nursing Case Management program provides home and community based case management
services for Oklahomans. The program assists older adults, individuals with chronic health
conditions or disabilities, and their caregivers/support system to achieve goals determined by the
individuals. The goal of the case manager is to assist these individuals to remain in their home with
appropriate services that ensure their health and safety needs are met. The program provides
guidance, advocacy and coordination for health care needs.

The program has expanded and partnered with other medical and health care providers along with
business communities to assist with case management services for individuals who have coordination
of care, transition of care or chronic care needs.

The OU Nursing Case Management program has over 40 professional providers and has offices in
Elk City, Lawton, Tulsa and Oklahoma City serving all 77 counties. For additional information, call
