Advantage Program

Care Management

A collaborative approach to assessing, providing, coordinating and monitoring healthcare services

The Advantage Program

The ADvantage Program is a home and community-based alternative to placement in a nursing facility to receive Medicaid-funded assistance for care.

The OU College of Nursing delivers care management services to ADvantage-eligible and approved elderly and disabled individuals (as determined by the Department of Human Services). The ADvantage Program offers an alternative to nursing facility care by making support services available at home to promote greater independence and a better quality of life.

Services include: case management, personal care and homemaking assistance, home-delivered meals, adult day health care, respite for caregivers, skilled nursing, environmental modifications, hospice, prescription medications, supportive restorative assistance, and specialized medical equipment and supplies. The ADvantage program also offers an Assisted Living Service Option and a Consumer Directed Personal Assistance Services and Supports (CD-PASS) when you qualify.

Am I Eligible for the Advantage Program?

Individuals qualify for the ADvantage program by meeting eligibility requirements that include both health and financial criteria. To see if you qualify for the ADvantage program call our intake staff or your local DHS office.
